Prairie Clinic, S.C. is an independent clinic offering Family Medicine, Pediatric, Obstetrics and Gynecology services. In addition to acute care and medical consultation, the clinic offers programs for Asthma, Anti-Coagulation (ProTime) and Diabetes. Some laboratory tests are completed on site while other laboratory tests are submitted to a reference lab. X-ray, ultrasound, and electrocardiogram are completed on site.

Pediatrics
Pediatrics is the medical specialty concerned with infants, children and young adults. A pediatrician's role includes preventive care along with the diagnosis and treatment of conditions and diseases in patients from birth through adolescence. Prairie Clinic schedules regular appointments for routine care and also provide urgent unscheduled care on a "work-in" basis.

A Physician Assistant (PA) is a nationally certified and state-licensed medical professional.
PAs practice medicine on healthcare teams with physicians and other providers.
They practice and prescribe medication in all 50 states, the District of Columbia, U.S. territories, and the uniformed services.
A Certified Nurse Midwife (CNM) is a licensed healthcare professional who specializes in women’s reproductive health and childbirth. In addition to attending births, they perform annual exams, give counseling, and write prescriptions.
My Patient Portal/ NextMD Web Portal
Prairie Clinic uses NextMD (also known as the NextGen Patient Portal) as our patient portal service. A patient portal allows our patients on-line access to their physician's office for secure messages, medication refill requests, records requests and even look up health information. Requests are typically answered with 2 business days.

NextMD offers a secure way to send messages, using 128-bit encryption, firewalls and secured systems. Once you sign up for NextMD, you'll use a username and password and a confirmation question to access your on-line account. When we send you information, you'll receive a notification e-mail, but you need to log onto the secure website (https://www.nextmd.com) to read your confidential information.

At Prairie Clinic, we try to schedule appointments for our patient's convienience, as well as for the efficient use of our providers' time.

When you come to Prairie Clinic for your annual physical, we ask that you fill out a history form to help keep your provider updated on your past and present medical history. The form for seeing our OB/GYN or Certified Nurse Midwife are a bit longer, as they need more indepth information. The forms are posted below:
Prairie Clinic is able to file the majority of insurance claims directly with your insurance company if we have received all the necessary information. When you have a change in insurance coverage, our office needs to know before services are provided, in order to file your claim more efficiently. When you check in for an appointment, please let the receptionist know of any changes. Please bring your insurance card with you. It is your responsibility to be aware of the coverage of your insurance plan. If you are a member of an HMO plan, Prairie Clinic must be selected as your primary care provider for you to be seen here.
Workmen's Compensation
It is the responsibility of the employee with a work-related injury or illness to inform the Clinic when the appointment is made or at the time of service. The Clinic will complete the form and forward it directly to the patient's employer or insurance company. For questions, ask for the Workmen's Compensation Specialist.
Medicare
All services provided by the Clinic Physicians are filed automatically with Medicare. Please let your doctor or nurse know if this is your first physical with Medicare, so we can process your claim properly. We accept assignment. For questions, ask for the Insurance Department.

How do I find out my test results?
We understand patients are anxious to receive test results. Most routine lab test results are received in the office and reviewed by the physician within a week, but some tests take somewhat longer.
If a test result is found to be a critical value, you will be notified at once by the provider. All other test results will be relayed to you by mail, patient portal or a telephone call from your provider's assistant.
If you have not received your test results within ten days, please call and leave a message requesting the results. Your provider's assistant will be glad to return your call.
Prairie Clinic is using a product called Medfusion (MyPortal) for secure communications with our patients. To enroll, contact your provider. Upon enrollment, you’ll receive instructions for how to complete that enrollment. Click to view Prairie Clinic's All NEW Medfusion Patient Portal FAQ
Patients who are already enrolled will need to re-enroll.
Prairie Clinic has a physician on-call 24 hours a day, 7 days a week. If your doctor is unavailable, the doctor on-call can answer your questions and care for any medical problems. Urgent care appointments are usually available during regular office hours. After Prairie Clinic hours, the doctor on-call can be reached by calling the Sauk Prairie Health Care at 643-3311.

To refill your prescription, please call your pharmacy and ask them to fax us the request for a prescription refill.
We receive many requests for refills each day, and the process takes time, (especially when your provider is not in the office). Please allow 48 hours for your request to be processed.

Statements are mailed every 4 weeks. The statement you receive is an exact copy of your account. You will not receive a statement if we are waiting for payment from your insurance company. After insurance pays on that date of service, you will receive a statement. We try to send everything to your insurance company as soon as possible, although your insurance company can take up to 6 weeks to pay on your claim.
